The Advance Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in the Anesthesiology Pain Clinic to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veteran Affairs mission. The NP provides advanced clinical practice, consultation, medication management, in office procedure, and management emphasizing a "Whole Health" approach, focusing on comprehensive care and coordination.
The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or programs as indicated by their position. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management.
Performs comprehensive evaluations, including physical examinations, ordering and interpreting diagnostic tests, order interventional procedures, and reviewing patient medical histories in person and through telehealth. Review and manage patient medication regimens for clinical effectiveness, drug selection, dosing contraindications, side effects, potential drug interactions, and therapeutic outcomes as required.
Formulates and implements personalized care plans utilizing pharmacological and nonpharmacological interventions. Prescribes pharmacologic and non-pharmacologic interventions Maintains accurate and timely documentation of pain management and therapeutic changes in the Electronic Health Record (EHR). Colloborates with multidisciplinary teams, including the Patient Aligned Care Team (PACT) providers, pharmacists, and specialists to provide comprehensive pain management.
Performs education to patients anti their families on pain management strategies, lifestyle modifications, self-care, a d Overdose Education and Naloxone Distribution (OEND). Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests and diagnostic studies. Follows care management as outlined in multi-service line agreements. Initiates referrals and consult to specialty services.
