The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. This job will close when we have received 50 applications which may be sooner than the closing date.

Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position. Note: All CRH Primary Care NPs will be required to travel to the VA Medical sites of care every 90 days for face-to-face encounters. If hired for locums-type services, in-person care at the patient site will be required for extended periods.
A CRH is a repository of clinical and administrative staff that serve all Veterans Affairs facilities within a Veterans Integrated System Network (VISN). Staff within these CRH are expected to provide interprofessional services to VISN facilities that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care.
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities with this position include, but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families.
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
