The VA Greater Los Angeles Healthcare System (GLAHCS), Sepulveda VA Medical Center, is seeking a highly skilled and experienced Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) for Community Care Service. The NP provides advanced clinical practice, consultation, and management; practices to the full extent of education, training, certification, and clinical privileges; and has prescriptive authority for an assigned patient panel or program.
Major Duties and Responsibilities include, but are not limited to: Independently manages a panel of patients with acute and chronic health conditions, emphasizing health promotion, disease prevention, and evidence-based practice. Develops and implements individualized plans of care to achieve optimal population health and disease management outcomes. Demonstrates leadership and collaborates effectively with members of the interdisciplinary healthcare team.
Orders and interprets laboratory tests, diagnostic studies, and other appropriate clinical evaluations. Prescribes pharmacologic and non-pharmacologic interventions within established scope of practice and clinical privileges. Initiates appropriate referrals and consultations to specialty services to ensure comprehensive and coordinated patient care.
Provides education and counseling to patients and their families regarding health conditions, treatment plans, medications, and disease prevention. Assesses patient response to treatment and adjusts the plan of care as needed to achieve optimal patient outcomes. Leads program development. Provides administrative support to service.
