Job Summary

The Iowa City Veterans Affairs Health Care System (ICVAHCS) is seeking a Full-Time, Advanced Practice Nurse - Home Based Primary Care to join the Rehabilitation and Extended Care Service Line at the VAMC in Iowa City, Iowa. Note: This is an OPEN CONTINUOUS ANNOUNCEMENT through December 15, 2026. The first referral cut-off is October 11, 2026, at 11:59 p.m. EST, with monthly cut-offs on the 1st.

Key Responsibilities

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.

This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.

Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies.

Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. All visits are conducted at patient's homes, might include assisted living facilities.

Other duties as assigned VA offers a comprehensive total rewards package: VA Nurse Total Rewards Work Schedule: Monday through Friday 7:30am-4:00pm. Alternative work hours, if desired, should be agreed upon at the time of hire. Telework: Not Available. Virtual: This is not a virtual position. Education Debt Reduction Program (Student Loan Repayment): This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
