The Geriatrics and Extended Care Service (GEC) provides care in several clinical venues including inpatient, outpatient, community, and research settings, including but not limited to: Community Living Center (CLC), Hospice and Palliative Medicine (Hospice House and outpatient), Geriatric Evaluation and Management Clinic (GEM Clinic), Home Based Primary Care, Inpatient and Outpatient Consultation Services (Geriatric and Hospice Services), Community Outreach, Education and Research Services.
Duties include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention, maximization of function, and/or assessing and honoring seriously ill Veteran's goals and preferences for care and quality of life using evidence-based practice.
Develops and implements an individualized plan to achieve optimal population health and disease management - Demonstrates leadership Works collaboratively multi-disciplinary team Orders and interprets diagnostic studies Communicates effectively with patients, patient caregivers, colleagues, and interdisciplinary team members Cooperates with nursing staff and other disciplines to provide patient care - Prescribes pharmacologic and non-pharmacologic interventions.
