The Advanced Practice Nurse - Certified Nurse Practitioner (NP) practices in Primary Care Service Residency Program Clinical Faculty to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.
Major duties include, but are not limited to: The primary clinical faculty is assigned to the residents for the entire year and provides clinical supervision, guidance, and mentoring. The clinical faculty will be responsible for structured and formal evaluations of the residents' competency, performance, and progress. The resident's continuity of care clinic is under the supervision and oversight of their clinical faculty member.
The residency program director will meet regularly with the clinical faculty to evaluate progress and ongoing professional development needs. The clinical faculty will be available to the residents as needed for mentorship and guidance. Residents, clinical faculty, and the program director will meet weekly as a group to provide support and guidance.
Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies.
Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
