The Advanced Practice Nurse (APN) - Cardiology functions as a provider, overseeing care provided to patients in the Cardiology Clinics and inpatient setting including but not limited to Interventional and EP Cardiology clinics.
This includes identifying the patient's medical problems through proper assessment, defining the medical management of these problems, determining need for consultation from medical, surgical, or psychiatric subspecialty clinics, and evaluating outcomes.
The APN - Cardiology practices in Cardiology Section, Medicine Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. Major duties include, but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families.
All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service.
