The main campus is in Hampton, Virginia with 4 community-based outpatient clinics in Chesapeake, Portsmouth and Virginia Beach, Virginia and Elizabeth City, North Carolina. This position is located Hampton Campus Prime 1 Clinic.
The PACT Assistant Nurse Manager is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.
Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. The Assistant Nurse Manager consistently practices customer service performance expectations consistent with the I CARE (Integrity, Commitment, Advocacy, Respect and Excellence) Values of the Department of Veterans Affairs.
General Description of Assigned Duties: Promptly reports clinic and staff changes, unusual events and safety to Nurse Manager. Able to provide patient care as outlined by the Nursing staff job descriptions for the unit/clinic. Assists in the hiring/selection process for Nursing staff. Supports Nurse Manager in collecting and preparing evidence for disciplinary actions for Nursing staff.
Identifies potential crisis situations and responds effectively in emergency/code situations in the clinic. Functions as a resource person to nursing and other personnel for clinic practices and patient care issues. Collaborates with the Nurse Educator in preparation and delivery of education/information to staff. Works with Nurse Manager in mentoring new staff and students in clinical and professional skill development acting as a role model.
Audits charts for complete documentation as required for specific clinical Outpatient setting. Proficient using VA EMR (electronic medical records) and nursing specific documentation. Monitors for timely completion of annual and intermittent competencies for employee compliance such as RQI, PMDB, and clinic specific requirements. Assists in the monitoring status of Nursing licenses (LPN, RN).
Demonstrates proactive approach and demonstrates Servant Leadership style toward teambuilding and fostering a positive work environment and demonstrates Servant Leadership style toward teambuilding and fostering a positive work environment. Demonstrates ability to surrogate coverage and perform other PACT staff members as needed. Provides knowledge and assistance to pull data specific to PACT and panel management.
Provides and documents patient education specific to disease states. Assists in developing and implementing plans to meet learning needs and skill development of Nursing staff. All other duties as assigned. Preferred Experience: At least 2-3 years of Primary Care or PACT experience A current, valid state-issued Driver's License is required for this position.

Nursing Bridge Programs: Nursing bridge programs are accelerated nurse education programs which rely upon existing nursing knowledge and education. Graduates from nursing bridge programs that meet accreditation requirements specific grade level requirements in this standard must also be met in addition to the following:(a) Master's Level Bridge Program Education Appointments Prior to Graduation.
Individuals attending a master's level bridge program in nursing who achieve the level in their education in which they have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program.
Upon achievement of a State license, the individual may be appointed on a temporary basis for any period up to three years contingent upon full completion of the professional nursing education program, if education verification provided by the school supports the Bachelor of Science in Nursing (BSN) equivalency.
Upon successful completion and graduation from the bridge program, the Nurse may be converted to a permanent appointment under the provisions of section 7401(a)(1) of title 38. Failure to complete the bridge program on which the temporary appointment was based will result in termination of the appointment.
Degree in a Related Field: A graduate level or doctorate degree in health care/management related fields from an institution accredited by the State and approved by the Department of Education at the time the candidate completed the program may be credited as specifically indicated in grade requirements. The graduate level or doctorate degree and coursework must provide evidence in support of the nursing role and duties associated with the assignment and grade level.
Foreign education programs/degrees are not creditable as related degrees. Dimension Requirement for Nurse III: Nurses at Nurse III must meet ALL the following dimension requirements to qualify for Nurse III. Practice. Demonstrates expertise in professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation and evaluation) with improved evidence-based and/or data-driven outcomes beyond the immediate practice setting.
Veteran/Patient Driven Care. Leads initiatives to improve Veteran/patient experience. Satisfaction and/or safety beyond the immediate practice setting. Leadership. Communicates and collaborates as an effective leader of the inter-professional team, at the expert level, with improved evidence-based and/or data-driven outcomes beyond the immediate practice setting. Professional Development.
Enhances knowledge base and practice evaluation, at the expert level, with improved outcomes beyond the immediate practice setting. Evidence Based Practice/Research. Leads the identification and integration of evidence-based practice/research with clinical expertise to improve quality of care and resource utilization at the expert level with improved outcomes beyond the immediate practice setting.
