Job Summary

The Marion VA Medical Center Behavior Health Service line is seeking an RN to serve in the role of RN Care Manager for the Behavioral Health Services. The Registered Nurse in this position provides health care services and care management to high-risk patients with in the Outpatient Services.

Major Duties

The RN is accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking an individual who possess good organizational skills which enable completion of work, demonstrated ability to assume leadership in a patient centered program and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. He/she assesses, plans, implements and evaluates patient care needs assuring quality of care. Provides professional nursing care that includes, but is not limited to: o Case Management. o Patient Assessments. o Participation in interdisciplinary team process. o Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care). o Completes and submits clinical notes. o Observes patient for changes in conditions and reports appropriately. o Provides education instructions to patients and their designated significant other. o Performs other professional nursing duties as assigned and within their scope of practice. This position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital. The incumbent may be required to rotate tours and duties during low/high census or complete administrative tasks established by the immediate supervisor.
