Duties and responsibilities of the Cancer Care Navigation (CCN) Registered Nurse (RN) include, but are not limited to: The Cancer Care Navigation (CCN) RN works collaboratively with the local and network multidisciplinary CCN, under the nursing direction of the Nurse Manager, CCN to ensure that patients have timely access to cancer care and psychosocial support.
The CCN RN identifies and eliminates barriers care, coordinates care, and provides case management across the cancer continuum and through transitions in care. The CCN RN facilitates system-wide improvements at Bay Pines VA Health Care System and across VISN 8 to improve cancer care delivery and patient outcomes.
The CCN RN supports patients at risk for barriers or delays in accessing cancer care (e.g., patients traveling for care from outside the catchment area and patients with a complex, multimodality, multi-specialty treatment plan). Collaborates with the Nurse Manager, to support effective implementation and sustainment of the local Cancer CareNavigator program.
Provides local support in improving patient care, maintaining standards of care, and educating staff/providers in a variety of settings.
