Job Summary

The Cardiology Care Coordinator Registered Nurse position delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.

Key Responsibilities

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.

This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.

Duties may include, but not limited to the following: Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.

Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team. Evaluates care and outcomes to ensure timely and appropriate provision of services. Advocates for holistic care through patient/family education and provider interaction. Implements an educational plan to meet the patient care needs. Interacts as part of the inpatient and outpatient multi-disciplinary care team.

Utilizes databases to provide population management to track patient care through all episodes of care. Analyzes and manages laboratory and radiology results and provides information to providers.
