Duties and responsibilities of the CAREGIVER SUPPORT PROGRAM REGISTERED NURSE include, but are not limited to: Caregiver Support Program Staff Registered Nurse (RN) is responsible for providing competent, evidence based outpatient care to assigned patients.
Assumes responsibility for the coordination of care focused on patient transition through the continuum of patient care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. The Caregiver Support Program Nurse reports to the Chief of Geriatric Extended Care in Nursing with programmatic report oversight from Chief, Care Management and Social Work at the facility.
Provides clinical evidence-based services and interventions for the caregiver and Veteran; education and consultation for the caregiver, Veteran, and staff on caregiver issues; community outreach; resource development; continuous quality improvement activities; and the evaluation of caregiver issues. Must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.
Administratively responsible for the clinical programming and daily operations as assigned, as well as, for themanagement of data related to the program at the facility level. Responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. Executes position responsibilities that demonstrate leadership, experience and creative approaches to management of complex patient care.
Drawing upon his/her clinical and administrative expertise and knowledge of health care programs, and in collaboration with the healthcare team, responsible for program functions that support the clinical, research, and education missions of the VA.
Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.
