Job Summary

The Iowa City VAMC is currently recruiting for a Case Management (CM) Registered Nurse (RN) - General Surgery/Plastics. The CM RN - General Surgery/Plastics provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.

Key Responsibilities

Major duties include, but are not limited to: Provides and supervises nursing care using all steps of the nursing process. Assesses, plans, implements, and evaluates age-specific patient care. Coordinates care focused on patient education, self-management, and satisfaction. Ensures care delivery follows established policies, procedures, and guidelines. Collaborates with interdisciplinary team members to influence care outcomes.

Actively participates in unit/program quality improvement and customer service initiatives. Manages patient care, coordination, and discharge/disposition planning for inpatient and outpatient settings.- Assists Veterans, families, and caregivers in navigating complex health care needs and service options. Supports care for patients with acute, chronic, complex, catastrophic, or life-threatening conditions.

Coordinates with multiple providers across all levels and sites of care. Addresses psychosocial, nursing, and medical needs through interdisciplinary care planning. Evaluates care processes and outcomes to ensure timely, appropriate services. Demonstrates leadership in improving holistic, evidence-based care. Provides peers with constructive feedback and fosters a supportive professional environment.

Contributes professional nursing perspective in interdisciplinary discussions. Partners with colleagues to drive change and achieve optimal patient outcomes. Shares case management knowledge to support safe, quality nursing care. Shares educational findings and participates in building an evidence-based practice culture. Questions clinical practices to support continuous improvement and evidence-based care.
