Job Summary

The Registered Nurse (RN) in this role is located in the Community Living Center (CLC) of the CHYVAMC.

Key Responsibilities

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.

Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. The Community Living Center (CLC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care.

Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.

Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level. The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score.

The Registered Nurse (RN) in this role is located in the Community Living Center (CLC) of the CHYVAMC. This person performs the following roles: Charge Nurse, Medication Administrator and provider of clinical treatment for veterans on the unit. The incumbent follows provider orders and is competent to start and maintain Intravenous therapy.

As Charge Nurse, maintains the daily flow of the unit, creates and issues daily reports, verifies orders, communicates with providers, families and visitors. The RN is expected to assign and supervise the work of licensed practical nurses and certified nurse aides to ensure the flow of the unit meets the clinical requirements and is performed safely to meet quality standards.

The CLC RN is responsible for ensuring artifacts of cultural change promote residents rights and a home like environment. The RN has assigned collateral responsibilities which may include infection control, wound monitoring and reporting, staffing methodology, Inter-Disciplinary Treatment Team (IDTT) meetings, restorative, reassessment tracking, and or safe patient handling.
