Job Summary

The Clinical Pharmacy Specialist (CPS) is a professional with advanced knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. The primary responsibility of the CPS Oncology is the provision of evidence-based pharmaceutical care services through direct and indirect interaction with patients and the healthcare team to support continuity of care.

Key Responsibilities

The duties of the Clinical Pharmacy Specialist include, but are not limited to the following: Clinical Fully utilizes the highest level of clinical privileges for CPSs to manage assigned patients. The CPS provider is an integral team member providing CMM expertise to reduce medication related problems, errors, and morbidity in our Veterans and as part of the Oncology team in delivering evidence-based, safe, efficient, and cost-effective care.

Directly assists clinicians as a drug information expert; assists in drug selection to achieve optimization of rational and cost-effective drug therapy by providing written and/or verbal consultation. Serves as a physician extender/independent practitioner in a clinic setting and independently manages pharmacotherapy.

Interprets drug history, medical history, allergies/adverse drug events, physical examination and ongoing patient specific monitoring of therapy and documents in the Electronic Health Record (EHR). Performs a medication reconciliation as a component of provider duties as appropriate. Serves as a pharmacy representative on pertinent committees as deemed necessary. Education Educates patients on the purpose, proper use, and expected outcomes of prescribed medications.

Serves as preceptor for pharmacy students and residents; designs and implements the clinical education programing for pharmacy clerkship in oncology. Maintains minimum ASHP requirements for preceptorship. Provides education and training support to other health care professional with an emphasis on pharmacotherapy including but not limited to formal and informal drug education, in-services, and continuing education programs.

Collaborates with the other clinical specialists in the provision of training programs and assists in the development of innovative training concepts aimed at improving the knowledge and quality of pharmacy and other clinical service staff. Administrative Participates in the Peer Review Program. Maintains records of clinical activities for quality assurance as directed. Collects interventional data regarding oncology surveillance.

Documents cost reduction/cost avoidance activities on a periodic basis. Develops and conducts drug usage evaluation in areas of needs from a patient outcome perspective and cost effectiveness. Manages the formulary and ensures its appropriate utilization by health care providers. Distribution Responsible for the safe and appropriate distribution of medications.

Ensure compliance with all business rules and regulatory policies with regards to medication labeling, inventory management, formulary management, PBM initiatives, equipment maintenance, and timeliness of service. Clarifies provider's orders through discussion with healthcare system staff.
