GS-13 Clinical Pharmacist Practitioner (CPP) The CPP delivers evidence-based comprehensive medication management (CMM) through direct patient care and is part of the interdisciplinary team. The CPP will incorporate a "personalized, proactive, patient-driven" approach to care that empowers and equips Veterans to take charge of their health and well-being and to live their life to the fullest in accordance with overall VA initiatives.

Core functions include face-to-face, VVC, telephone encounters with referred patients, for CMM in practice areas identified in their individual scope of practice. b.
Functions as an advanced practice provider for CMM services to achieve defined outcome, this includes design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. c. Functions as part of the interdisciplinary team to provide CMM which may include, but is not limited to: i. Prescribes, monitors, and adjusts medications. ii.
Obtains pertinent medical history and obtains accurate medication related history from patients and via chart review. iii. Obtains oral informed consent if ordering pharmacogenomic lab and documents this encounter in the electronic medical record iv. Orders lab testing when indicated to aid in the process of medication and dosage selection or deprescribing. v. Monitors, and interprets lab results and implements follow up on treatment actions as clinically indicated. vi.
Performs rating scales on pain control, symptoms and functional status vii. Queries and documents PDMP findings viii. Uses available population health tools to identify and address risk mitigation needs focused on safe pain medication care ix. Provides overdose education and prescribes naloxone when needed x. Identifies and reconciles drug-drug interactions. xi.
Designs patient-specific medication and dosage regimens based on a person's PGx profile and other pertinent factors, such as pharmacokinetic and pharmacodynamic properties of a medication, drug-drug, and drug-gene interactions, comorbidities, patient demographics, and laboratory data to optimize patient outcomes. xii. Provides resources and education that empower patients to make informed healthcare decisions about medication regimen. xiii.
Ensures referral of complex or unstable patients to higher level care, when needed. xiv. Communicate with team for next steps and coordination of care, when necessary. d. Performs limited external physical assessment and performs medication reconciliation in alignment with established policies or procedures. e.
Accepts urgent or same day patient visits including, but not limited to, patient medication review for polypharmacy, recent hospital or emergency department discharges. f. Follows up via face-to-face, telehealth, eConsult, telephone, secure messaging, and other modalities to provide patient care. g. Follows-up with patients on lab or test results to discuss the plan for therapy, i.e., changes in medication therapy, monitoring, and additional testing requirements. h.
CPP will determine if a referral to the physician is necessary when disease progression occurs, or adverse drug events require treatment interventions outside of the CPP scope of practice. Clinical- Indirect Patient Care a. Identify patients who may benefit from CMM through population management reports, dashboards, and during team huddles. b. Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. d. Monitors and reports drug errors, adverse drug reactions, allergies, and patient compliance issues.
Documents findings per facility procedures and in accordance with the VAADERS Adverse Drug Reaction (ADR) reporting program. e. Documents clinical interventions and patient care encounters in the electronic health record in a timely and professional manner as appropriate. f. Apply knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment or recommendation based on objective laboratory findings. g.
Conduct clinical review and adjudication of prior authorization requests for highly specialized specialty care medications for appropriateness and compliance with established criteria where applicable. h.Assists Pharmacy and Therapeutics committee with medication use evaluations and other safety initiatives where applicable. i. Promotes and monitors compliance for established drug therapy policies. j.
Works with providers to ensure compliance with national, VISN, and local initiatives.
