Job Summary

The Community Care Nurse Manager practicing in the Supervisory path is a nurse who provides indirect or direct nursing care to patients and families. The Nurse Manager utilizes the nursing process to assist, plan, implement and evaluate Community Care.

The Community Care Nurse Manager practicing in the Supervisory path is a nurse who provides indirect or direct nursing care to patients and families. The Nurse Manager utilizes the nursing process to assist, plan, implement and evaluate Community Care. The Nurse Manager provides supervision of clinical staff and has continuing responsibility for the following functions: 1.

Planning work to be accomplished by subordinates, setting priorities, preparing schedules for completion of work, 2. Assigning work to subordinates based on priorities considering the difficulty and the requirements of the assignments and the capabilities of subordinates, 3. Evaluating the performance of subordinates, 4. Making recommendations for appointments, advancements or reassignments of subordinates, 5.

Giving advice, counsel, or instruction to subordinate personnel on work and administrative matters, 6. Hearing and resolving complaints of subordinates and referring more serious complaints not resolved to a higher-level supervisor, 7. Recommending and/or taking disciplinary action where appropriate, 8.

Identifying developmental and training needs of subordinates and providing or making provision for such development and training (VA Handbook, 5007, Pan M Chapter 8) and other duties as assigned.

In addition, the Community Care Nurse Manger takes responsibility for role specific standards of care and practice, performance improvement, coaching staff, customer service and patient satisfaction, the work environment, patient and employee safety, and accreditation and regulatory standards.

He/she possesses intimate knowledge of Fee federal regulations, VHA guidelines, medical review criteria, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. The incumbent supports the medical center's mission by possessing and exemplifying the core competencies of the High-Performance Model.

The Community Care Nurse Manager actively develops and manages collaborative partnerships with physicians and providers both within and outside of VASDHS and other clinicians involved in health care delivery.

Preferred Characteristics MSN strongly preferred Experience supervising Registered Nurse's Experience with VHA Community-based care referral processes Evidence of self-direction Flexibility in schedule and assignments Willingness to cooperate as a member of a team and individually as the situation warrants Ability to communicate with people of varied educational and ethnic backgrounds Knowledge related to VHA guidelines and policy related to Community-based referrals and care delivery processes, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy preferred.
