This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Independently manages Veterans under discretionary contracts awaiting SVH placement.
Coordinates with SVH admission staff to ensure timely and appropriate placement. Reviews Veteran Directed Care consults, performs biopsychosocial assessments, determines eligibility, and completes required documentation. Provides expert guidance to Veterans, caregivers, and interdisciplinary teams. Uses the Case Mix Tool to determine service hours and collaborate with stakeholders.
Provides specialized case management for STR Veterans needing enhanced services, particularly those at risk of homelessness. Submits and monitors referrals to GEC administrative staff for financial authorization processing. Visits Veteran patients at CHNs to conduct thorough psychosocial assessments and make psychosocial diagnoses. Provides high-quality case management and care coordination to Veterans residing at CNH or instructional settings.
Independently works with Veterans and their families who are experiencing a variety of psychiatric, medical, and social utilizing individual/family counseling. Assists CNH staff with determining appropriate care planning including discharge planning as needed. Assesses CNH Veterans' functional status and fitness for continued placement. Address all CNH resident/family concerns identified.
Facilitates meetings with the CNH staff when appropriate to allay or address the VA, Veteran, or family concerns. Demonstrates an advanced understanding of the range of treatment and skills needed for adult patients related to physical limitations, psychological needs, and associated illnesses and conditions. Monitors and provides suicide risk assessments to those residents identified at risk.
Collaborates with other service providers in reassessing the Veteran's need for continued institutionalized care and/or works with the CNH discharge planner to coordinate movement to lower levels of care or home. Maintains communication with GEC supervisory or administrative staff, VISN, and national information as applicable and keeps abreast of changes to the program at the national level.
Acts as an advocate for the Veteran/family as well as advocating for new/additional services in the community. Processes alleged abuse cases according to established VA and community policy and keeps leadership abreast of any active abuse allegations. Provides education related to VA and community resources, entitlements, advance directives, and health care planning.
Provides Veterans and their caregivers with ongoing supportive counseling related to chronic/terminal illness and the need for institutional care. Coordinates care with CNH staff to assist Veterans and family members with grief and loss counseling. Establishes and maintains positive working relationships with employees, volunteers, and community partners.
Collaborates on performance improvement for the service and facility to ensure compliance with local and national regulations.
