This position will provide quality outreach health care services and community-based health promotion and disease prevention services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Incumbent will be required to provide medically related transportation for community members focusing on the elderly and adolescents that are recognized with lack of transportation resources. Incumbent's primary responsibilities are to provide home health visits to high risk elderly and adolescent clients in coordination with the Public Health Nursing and or Director of Nursing. Provides education on healthy lifestyle habits to all clients. Notify clients of scheduled medical appointments and follow-up from PHNs, Charge Nurse and referred providers appointments.
