The incumbent of this position will serve as a Health Technician and function as a member of the Diabetes Prevention and Education Program staff with primary functions being to assist staff with the preparation, implementation, and documentation of diabetes prevention activities in the community. This position is supervised by the Diabetes Program Manager. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
Develop and implement recruitment/retention plan in conjunction with other Diabetes Prevention Program grantees. Responsible for the daily management, maintenance, cleaning and sanitization of Fitness Center facilities and fitness equipment Member of an interdepartmental team to plan and implement the Diabetes Self-Management Education (DSME) curriculum to patients Identify and develop data required for use in the evaluation, management and direction of the grant program.
