This position is located in the Diabetes Branch within the Division of Public Health Services at the Crownpoint Service Unit at Crownpoint, New Mexico. The position promotes the improving the health of patients with diabetes and to assist with various health promotion activities. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

Retrieves medical charts to efficiently record and make appropriate disposition of the patient's record. Provides individual and group education as necessary and or requested an optical/ocular condition, and diseases.
Coordinates referral services and transmits patient information with other health care providers, schedules patients, arranges transportation, and any other activities to obtain evaluation and treatment results from referral sources.
