Job Summary

The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Geriatrics and Extended Care Service to provide a full range of health care services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.

Key Responsibilities

Major duties and responsibilities include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Visits patients in their homes in the community. Drives a vehicle to patient visits in the community and follows relevant policies and procedures regarding operating a government vehicle.

Assesses, identifies, and diagnoses patients' general medical problems and defines the medical management. Determines the need for specialty consultation. Develops and implements an individualized treatment plan to achieve optimal patient health outcomes. Adjusts the plan of care as patient needs change. Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members, as well as with other VA staff and other care providers in the community.

Engages constructively with other HBPC staff in a collaborative fashion during interdisciplinary team meetings to solve complex patient problems. Fosters open communication and strives to resolve conflicts promptly. Orders and interprets labs, tests, and diagnostic studies. Delivers direct nursing care in the home to patients.

Provides and/or coordinates skilled nursing services which may include but are not limited to: phlebotomy, medication management, wound care, catheter care, injections, and electrocardiograms Collaborates with homecare agencies, hospice and palliative care agencies, and other community care services.
