The PACT LPN functions as a member of the PACT team. The incumbent functions as a member of the nursing team and assists the Registered Nurse in providing holistic care to patients in the Primary Care Clinic settings. The LPN serves as a responsible member of the nursing team and interacts with patients, family members, professional and supportive personnel who provide patient care, and with members of other services.

Demonstrates active collaboration and communication with Core Teams (Patient, Primary Care Provider, RN Care Manager and MSA and the Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient). Performs screening for Telehealth visits and other Ancillary services. Primarily work in the ambulatory setting within specialty and primary care within the Finger Lakes Health Care System.
A basic understanding of the total patient program. Understanding of simple scientific facts and principles. A broad spectrum of technical skills. Adaptation of procedures to meet individual patient needs. Recognition of the relatedness of patients' behavior and physical condition.
