The Licensed Practical Nurse Ambulatory Care (Intermittent) serves as a member of the multidisciplinary health care team with selected patients and provides care to them on a regular and recurring basis. The LPN completes assigned duties and responsibilities, which involve performing standard and specialized practical nursing activities and which require broad work experience NOTE: This is an Intermittent Position.
The Licensed Practical Nurse (LPN) performs recurring duties that are carried out independently. The LPN recognizes the need for and seeks guidance when necessary to ensure quality patient care. The LPN administers medication, which implies knowledge of proper techniques and procedures. The LPN also documents observations, treatments, procedures, and conditions in the computerized patient record and reports them verbally to the appropriate person. Duties include but are not limited to: Observers changes in patient's/resident's condition, (e.g. behavior shifts, mood changes skin coloring, appetite, etc). Documents care provided and observations on appropriate tools or forms. Assures receipt of adequate nutritional intake either through observation, feeding, or properly positioning and checking placement of the feeding tube for the patient/resident. Administers medications, stoma care (e.g. tracheotomy, suctioning, colostomy), enemas, skin care, simple dressings, braces and prosthetic devices. Monitors intake and output, according to established policy and procedure. Recognizes the patient's/resident's autonomy and promoting independence by means of respect, limits setting, and individualized treatment. Reinforces age related healthcare education for patients/residents and or significant other. Integrates and applies pertinent aspects of the Outpatient Clinic performance improvement program into his/her professional practice as applicable. Schedule appointments for patients in collaboration with team members to ensure that all patients are scheduled within time constraints established by ACA guidelines and VHA policies especially adhering to priority access for service connected and OEF/OIF veteran populations.
