The Licensed Practical Nurse GS-7 serves as a member of the nursing team in the ambulatory setting (PACT/Specialty) within the Finger Lakes Health Care System.

Assisting in the collection of objective and subjective data relative to more complex patient situations. Participating in patient centered conference/shift reports, and contributing information for the development and modification of the patient care plan.
Anticipating a clean, safe and therapeutic environment and taking appropriate actions. Maintains & promotes safe waking practices in patient care. Performing aspects of nursing care in a variety of situations to meet patients' needs. Assisting with monitoring prescribed therapies such oxygen and IV therapy.
Reporting patient status to a registered nurse and documenting information in the medical record including current status and/or subtle changes in patient's physical, psychosocial and spiritual status. Care and Case management of Veterans established care plan. Includes: post hospitalization follow up, reporting any changes to teamlet necessary for care plan adjustments.
Ability to observe, identify and respond to the patient's needs for care, including medication, equipment assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN. Orienting all new LPNs.
Conducts daily/weekly/monthly clinical and environmental tracers per policy.
