Job Summary

This position is located at the Akron VA Clinic Community Based Outpatient Clinic in Akron, OH.

VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Duties include, but are not limited to: Conducts independent home visits for Veterans with complex medical and psychosocial needs, delivering direct nursing care and providing education to patients and caregivers in alignment with the individualized care plan established by the RN Case Manager.

Collaborates with the Home Based Primary Care (HBPC) Interdisciplinary Team including the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider. Collaborates with services internal and external to the VA to manage the care needs of the patients.

Provides patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals. Supports additional teams by completing home visits as needed based on patient care demands. Demonstrates strong prioritization skills in determining the order of patient visits according to acuity and need.

Utilizes Home Based Primary Care (HBPC) policies and procedures to provide autonomous, high-quality care in the home setting. Possesses the knowledge and skills to carry out more specialized patient care activities. Knowledge of program regulations and is able to carry out objectives of the Nursing program. Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur.

Work Schedule: 08:00am-04:30pm, Monday-Friday Compressed/Flexible: Not Available. Recruitment/Relocation Incentives (Sign-on Bonus): Not Available. Permanent Change of Station (Relocation Assistance): Not Available. Pay: Competitive salary and regular salary increases.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not Available.
