The Licensed Practical Nurse is located in the Care Navigation Center (CNC) Community Care section, organizationally aligned under the Deputy Chief of Staff at the Chillicothe VA Medical Center. The Licensed Practical Nurse is responsible for the administrative management of Community Care consults by coordinating referrals for Chillicothe VA Medical Center and associated Community Based Outpatient Clinics (CBOCs).

Provides purchased care and community care education to providers and staff, on an as needed basis. Acts as a liaison between Chillicothe VA and other VA and community facilities to ensure timely exchange of information related to consults and/or referrals. Scheduling appointments, interpreting, and verifying provider orders/consultations in accordance with Veteran Health Administration (VHA) national scheduling guidelines, scheduling, canceling, and re-scheduling patient's appointments and/or consults, entering no-show information, monitoring inpatient and outpatient appointments for areas of responsibility and preauthorization requirements for specific coverage. Monitors consult timeframe compliance. Reviews records for accuracy and completion. Collects data for reporting and dissemination to various programs, and manages medical administrative functions. Monitors progress of all open community outpatient consults within the medical center and corrects consult transmission errors and incorrect provider assignments. Utilizes multiple software applications to obtain information required to complete reports. The work requires long periods of utilizing a computer keyboard and up to two monitors, completing phones calls and electronic charting. Must have good hearing, eyesight, and physical dexterity. Specific vision abilities required by this job function include but are not limited are close vision, distance vision, peripheral vision, depth perception and ability to adjust focus Must be clean and neat in appearance. Must be emotionally stable and cooperative. Work is accomplished predominately in the Care Navigation Center. The physical surroundings involve working with Veterans, family members and surrogates to allow choices for ordered care in the community. The work is predominately performed in an office setting and involves the normal risks and discomforts typical of an office. The work area is adequately lighted, heated, and ventilated. Special correct ergonomic precautions must be taken with sitting and typing. The incumbent will follow safety precautions are necessary. Since work requires interaction with patients, employees may be exposed to communicable diseases. Additionally, incumbent is at times confronted by dissatisfied patients or family members, special violence prevention in the workplace safety precautions are required.
Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the National League for Nursing Accrediting Commission, 61 Broadway, 33rd Floor, New York, NY 10006. Exceptions include: Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
