The incumbent is a professional social worker assigned to the Transitional Care & Specialty Social Work section of Social Work Service. This social worker has responsibility for the provision of social work within assigned segments of the Community Nursing Home (CNH) program and the Adult Day Health Care (ADHC) program.
Total Rewards of a Allied Health Professional Duties include but not limited to: Advocates for the Veterans in the care of these agencies with the goal of helping the Veteran stabilize within the least restrictive environment.
Manages the daily operation of the ADHC program including responding to ADHC referrals, completing ADHC referrals in referral management, communicating with the ADHC facilities regarding authorizations and collaborating with Community Care in regard to authorizations. Assists with case managing the CNH Veteran participants and coordinates all activities related to this program. Processes referrals for the ADHC program in the Electronic Health Record (EHR).
This includes working with the Community Care team on providing authorization to the community agencies. Schedules annual visits with each individual ADHC community agency.

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
