Job Summary

A housing subsidy of up to $900 per pay period may be available. This position is located at the Blackfeet Service Unit, Division of Nursing Services within the Indian Health Service. The primary purpose of this position is to provide advanced professional nursing case management and care coordination services to eligible American Indian and Alaska Native beneficiaries. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

Major Duties

Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Manages an assigned caseload of patients with chronic or recurring health conditions that require case management services. Develops and implements individualized, patient-centered care plans in collaboration with patients, families, and the healthcare team. Participates in discharge planning activities for assigned patients, including identifying discharge needs, coordinating post-discharge services, and providing patient and family education. Coordinates care across providers, disciplines, and care settings to ensure continuity and prevent fragmentation of services; Provides direct professional nursing care to assigned patients with chronic and recurring health conditions.

Education
