*** Recruitment or Relocation incentive may be authorized for eligible, highly qualified candidates! *** The Housing and Urban Development- Veterans Affairs Supportive Housing (HUDVASH) Registered Nurse (RN) Case Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patent management. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Major duties include but are not limited to: Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care. Develops a plan of care using information from the patient, family and provider. Identifies high-risk patient populations and transitions to appropriate discipline. Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients. Collaborates with interdisciplinary team members to develop and implement the Veteran Housing Stability Plan. Contributes to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors. Provide recover-oriented and Housing First models, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of function. Addresses psychosocial, as well as nursing and medical needs of patients and their families/ caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates HUDVASH outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level.
