Leads and coordinates contact investigations and outbreak responses, conducting field visits to assess, screen, treat, and manage individuals, families, and communities. Plans, implements, and evaluates communicable disease programs; coLlaborates with Tribal, local, state, and federal partners; and coordinates outreach clinics and prevention activities.
Maintains disease registries, analyzes trends, tracks testing and treatment outcomes, and prepares required surveillance reports to support program effectiveness and public health action. Provides education and training to staff, patients, and community partners; offers clinical guidance; leads care coordination efforts; and supports quality improvement and evidence-based practices.
