This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Tsaile, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. Provides skilled direct nursing care across age groups, including triage, emergency treatment, clinical assessment, nursing interventions, and maintenance of accurate health records. Coordinates multidisciplinary services and continuity of care by collaborating with patients, families, providers, specialty services, outside facilities, transportation resources, and other healthcare agencies.
Provides patient and family education, counseling, and preventive health support using culturally appropriate approaches to promote self-care, disease prevention, treatment adherence, and improved health outcomes. Supports quality improvement, patient safety, and administrative/community outreach activities through documentation review, performance improvement efforts, staff and community education, and collaboration with the multidisciplinary healthcare team.
