Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information of changes to clinical plan between patient and provider/care team. Develop and maintain patient registries based on disease, health condition and/or health prevention initiatives. Assist with triage of patients, including triaging telephone calls and walk-in patients. Prepare patients for tests, exams and treatments. Coordinate appointments and procedures for patients who need further care or continuity of care at other facilities. Provide counseling and teaching to patient and families on preventive, curative and rehabilitative measures involved in health care.
