Job Summary

The Registered Nurse (RN) provides patient-centered, evidence-based nursing care within Community Health/Case Management. The RN assesses, plans, implements, and evaluates care for Veterans with complex health care needs and coordinates services across inpatient, outpatient, and community settings.

Key Responsibilities

The RN collaborates with Veterans, families, caregivers, and interdisciplinary health care teams to facilitate appropriate care, promote patient education and self-management, and ensure continuity of care throughout the health care continuum. The RN utilizes professional nursing knowledge, clinical judgment, critical thinking, and evidence-based practice to manage complex patient care needs and improve patient outcomes.

Depending upon the level of assignment, the RN may also demonstrate progressively responsible leadership through evaluation of care outcomes, quality improvement activities, education and knowledge sharing, and participation in the development and improvement of clinical practices, policies, and procedures.

Major duties and responsibilities include, but are not limited to: Performs comprehensive nursing assessments and develops, implements, and evaluates individualized plans of care based on patient needs and age-specific considerations. Provides patient management, care coordination, and discharge/disposition planning for Veterans receiving care in inpatient, outpatient, and community settings.

Assists Veterans, family members, and caregivers in identifying and accessing appropriate health care services and resources to address complex medical, nursing, and psychosocial needs. Coordinates care with multiple providers and services across various levels and sites of care to promote continuity and timely delivery of services. Provides patient and caregiver education to promote self-management, informed decision-making, and improved health outcomes.

Collaborates with interdisciplinary team members in the planning, delivery, and evaluation of patient care. Evaluates the effectiveness and outcomes of care and identifies opportunities to improve the quality, safety, efficiency, and appropriateness of services. Applies professional nursing knowledge, clinical judgment, critical thinking, and evidence-based practices when managing complex patient situations.

Participates in quality improvement activities and initiatives designed to enhance patient care and clinical outcomes. Questions and evaluates clinical practices to support evidence-based care and continuous improvement. Provides professional nursing knowledge, guidance, education, and constructive feedback to peers and other health care professionals. Promotes a safe and supportive environment that contributes to the professional development of health care staff.

Demonstrates progressively responsible leadership in the management and coordination of complex patient care. Participates, as appropriate to the level of assignment, in the development, implementation, evaluation, and revision of clinical policies, procedures, guidelines, and evidence-based practices. Performs other duties as assigned and may provide coverage or rotate among supported clinical areas based on organizational needs.

NOTE: This position is not approved for telework and staff is required to be on onsite in the clinical area of assignment. This is a VA-NYHHCS Position. This position supports all 3 campuses and may require rotation as needed.
