Job Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Key Responsibilities

Roseburg is the heart of the scenic and recreational paradise known as the Land of Umpqua. This land of authentic outdoor adventures and experiences is bordered by picturesque rivers and lush valleys. Roseburg, centrally located in the of Douglas County, Oregon is known for its 359 miles of streams along the north and south forks of the Umpqua River.

The Roseburg area reflects small-town values, has dedicated business leaders and a strong education system from K-12 to our local community college. The community possesses a hardworking, well-trained workforce. These characteristics have proved instrumental in drawing new residents and companies to the region, while keeping families here for generations.

Functions or Scope of Assigned Duties include but are not limited to the following: Direct Patient Care: Administer medications, perform comprehensive assessments, wound care, lab draws, IV medications, and skilled nursing services. Communicate with Veterans' families regarding care plans, progress, and concerns. Care Coordination Support daily unit huddles, team meetings, and performance improvement projects.

Coordinates care from admission to discharge and gives input to each individualized care plan. Collaborate with interdisciplinary teams to optimize care and outcomes. Quality & Safety: Ensure compliance with quality improvement measures, participate in daily unit huddles, and implement action plans to meet performance measures. Adhere to safety, infection control, and environmental policies.

Team Collaboration: Work closely with Nurse Managers, Assistant Nurse Managers, and interdisciplinary teams to organize daily workload, review staffing needs, and control personnel assignments. Cross-cover to other units and act in broader roles as needed. Education & Training: Participate in orientation and ongoing staff and student educational .programs; provide staff education regarding end-of-life care and department outreach.

Documentation & Reporting: Maintain accurate records in the Electronic Health Record (EHRM), conduct chart reviews/audits, monitor for changes in condition, and report on psychotropic medication reviews.
