About the Position: This position is located at Evans Army Community Hospital, Fort Carson, Colorado. This is a Direct Hire Solicitation. A recruitment or relocation incentive MAY be authorized. Key Responsibilities
Develop clinical case management, care coordination and/or discharge planning protocols and procedures to facilitate quality care, healthcare coordination and collaboration, maximize MTF resources, and improve patient outcomes.
