This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The Mobile Medical Unit (MMU) Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care.
This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Responsible for delivering and coordinating care as a member of the MMU team when off station at rural sites and as a member of PACT when on campus. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Coordinates MMU travel schedule with rural clinical sites, at Veterans Affairs (VA) Stand-Downs, and for any special events. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care Department and facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations. Responsible for the documented PACT outcomes at the program or service level. PACT program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
The role of the Mobile Medical Unit/Telehealth RN is to provide primary care (PC) and mental health (MH) services, laboratory services, specialty Telehealth services and Tele-retinal imagining to Veterans in rural communities in Utah, Colorado (CO), and Western Colorado Health Care System (WCHCS) catchment areas.
The nurse in this position is responsible for scheduling appointments, completing clinical reminders, advocating for the Veteran, and working jointly with the PC and MH providers to provide care to the Veteran. The nurse plays a pivotal role in building relationships with community resources such as Veteran Service Organizations (VSO) in Utah, CO, and other catchment areas.
