The VA New York Harbor Healthcare System's Nursing Service is seeking an experienced and dynamic Registered Nurse (Outpatient Staff). Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components.
Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
Major duties include, but are not limited to: The Nurse III Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care Department and facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals.
Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations. Responsible for the documented PACT outcomes at the program or service level. PACT program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility. Will perform additional duties as assigned.
This position is not approved for telework and staff is required to be on onsite in the clinical area of assignment. This is a VA-New York Harbor Healthcare System Position. This position supports all 3 campuses and may require rotation to other campuses as needed. The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).
