Job Summary

The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Community Living Center (CLC) aligned under the Geriatrics, Rehabilitation, & Extended Care (GREC) to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.

Key Responsibilities

Roseburg is the heart of the scenic and recreational paradise known as the Land of Umpqua. This land of authentic outdoor adventures and experiences is bordered by picturesque rivers and lush valleys. Roseburg, centrally located in the of Douglas County, Oregon is known for its 359 miles of streams along the north and south forks of the Umpqua River.

The community possesses a hardworking, well-trained workforce. These characteristics have proved instrumental in drawing new residents and companies to the region, while keeping families here for generations.

Functions or Scope of Assigned Duties include but are not limited to the following: Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services.

Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
