A Career At VA Salt Lake City Health Care System Can Take You Places (youtube.com) Major Duties: Duties and responsibilities include, but are not limited to, the following: Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement an individualized plan to achieve optimal population health and disease management. Demonstrate leadership and work collaboratively with the interdisciplinary team. Order and interpret labs, tests, and diagnostic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families. Obtain and document relevant health and medical history. Document in Computerized Patient Record System (CPRS). Perform a physical examination based on history. Order and interpret diagnostic tests. Formulate the appropriate differential diagnosis based on history, physical examination and clinical findings. Establish a plan of care to achieve the identified medical and health goals. Prescribe medications which may include controlled substances per licensure. Identify patient education needs and provide for relevant education. Initiate referrals and/or consultations as appropriate to achieve an interdisciplinary plan of care including referrals to community agencies. Determine effectiveness of the plan of care through documentation of client (patient) care outcomes. Proactive in identifying potential emergency/crisis situations and providing appropriate interventions. Perform procedures within the approved scope of practice and based on documented competencies. Perform diagnostic and stabilization procedures based on ACLS protocol including oral endotracheal intubation/airway management, needle thoracostomy and cricothyrotomy. Place and maintain Peripherally Inserted Central Catheters (PICC), including de-clotting procedure. Place and maintain enteral feeding catheters. Place, maintain and manage internal jugular, subclavian and femoral central lines. Place, maintain and manage percutaneous arterial lines. Place and manage Swan Ganz catheters. Place pleural pigtail or chest tubes. Rewire central line catheters (Cordis to triple lumen). Remove of chest tubes (pleural and mediastinal), epicardial pacer wires, and femoral arterial sheaths, including IABP catheter sheaths.
