Job Summary

This position is located at the Northport VAMC within the home based primary care (HBPC) department. The Staff RN functions as a care manager and is responsible for delivering direct and indirect care for Veterans in the home setting.

Key Responsibilities

General Description of Assigned Duties: Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice in providing care to clients with basic or complex needs. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Administers medications and procedures per established policies and guidelines. Influence care outcomes by collaborating with members of the interdisciplinary team. Functions or Scope of Assigned Duties: The Registered Nurse is responsible for developing and coordinating the patient's plan of care to include ongoing assessment, and re-assessment, care planning, care coordination, monitoring and evaluating clinical outcomes.

Provides individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. Provide intensive care management linking Veterans with resources within and outside the VA and coordinating comprehensive care with VA and community providers.
