The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse assigned to Community Care Services as Case Manager/Care Coordinator.
The VA Specialty Care Referral Manager, provides direct staff support as well as oversight to the VA Specialty Care program in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and services to improve the quality of patient care and the appropriateness and timeliness of services provided.
The VA Referral Management Coordinator facilitates and coordinates services between providers of the VAAAHS and non-VA providers in the local community for outpatient and inpatient care that cannot be provided by the VA or cannot be provided due to geographical access. The incumbent will process consults utilizing clinical judgment, VARCI processes, and VA resources and guidelines.
The Referral Management Coordinator facilitates and coordinates services between providers of the VAAAHS and non-VA providers in the local community for outpatient and inpatient functions independently within his/her scope of practice and demonstrates clinical decision making capacity.
