The NA functions as a member of the nursing care team and assists higher graded NAs, licensed nursing staff, and other members of the care team in the care of patients/residents receiving outpatient care, home or community living care, or other patients who are in acute, sub-acute, or chronic states of illness. The Nursing Assistant in this role provides care under supervision and refers issues/questions to supervisors, higher graded NAs, or other healthcare professionals.

Initiate and/or actively participate on a unit level quality or process improvement project/initiative with demonstrated outcomes. Education: Carry out teaching activities as assigned and according to pre-established teaching plan. Reinforce patient education activities. Reusable Medical Equipment: Be responsible for knowing what reusable instruments, equipment, medical devices or supplies are used (inventory) within assigned area. Be familiar with which items are reprocessed locally (on unit) and/or which items are sent to sterile processing and/or SPD for reprocessing and/or sterilization. If items are to be transported for reprocessing, he/she/they must be aware of the initial decontamination steps that must be taken prior to transport of item and the appropriate method of transport. GS-4: Clinical Nursing Practice: Assists with admission, discharge, and transfer of patients. Completing duties as assigned and under the supervision and direction of the Registered Nurse (RN) and/or Lead Licensed Practical Nurse (LPN). Give bed bath, tub and shower baths, shampoo hair, and assist patients with grooming. He/She/They will consistently promote patient independence, rehabilitation and restorative nursing as the patient plan of care dictates. Administration: Review contributions to nursing care and establish goals for personal/professional development with assistance of Nurse Manager or Nursing Education, etc. Exercise economic use of supplies and apply adequate safeguards to prevent misuse and loss. Research: Participate in the accurate collection of data as instructed by the Nurse Manager or designee. Participate in Performance Improvement activities at the unit level. Education: Receive education related to special knowledge and skills needed to care for the geriatric patient annually at the unit level. Reusable Medical Equipment: Have documented training in the setup, reprocessing and maintenance of specific equipment. Validation of competency will be done, minimally, on an annual basis. Will suspend the reuse of any RME if he/she/they suspect that the device was not properly cleaned, disinfected or sterilized according to the SOP and manufacturer's guidelines. GS-5 Clinical Nursing Practice: Respond to patient calls for assistance in a timely manner. Ensure proper identification of patient per policy before administering care. Administration: Report to the Nurse Manager or designee any failure to comply with established policy or procedure. Assist with answering the phone conveying timely messages to the appropriate staff, placing calls/pages as needed. Research: Participate in the accurate collection of data as instructed by the Nurse Manager or designee. Participate in Performance Improvement activities at the unit level. Education: Demonstrate accountability for on-going education requirements as required by facility and assigned unit. Reusable Medical Equipment: Will have an understanding of the importance in reprocessing of RME and the significant impact on our Veteran population through the risk of infectious disease.
