These positions are located in Patient Care Services at the Lieutenant Colonel Charles S. Kettles VA Medical Center in Ann Arbor, Michigan. The Nursing Assistant will assist licensed nursing staff in the care of patients/residents receiving preventive, acute, sub-acute chronic, maintenance, and hospice care.
The distinguishing factor is that patient/resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements.
Major duties of this position will include, but are not limited to: Perform and document prescribed medical treatment and interventions for patients, such as diabetic urine testing and blood testing, and simple dressing changes. Monitoring patients according to levels of observations i.e. (one to one suicide, continuous observation, q 15 minute checks, and hourly rounding.
Monitoring Veterans who may be at risk for harming themselves and or others according to established policies and procedures and appropriately refers issues to the charge nurse or other health care professional. Utilize principles of proper body mechanics while caring for patients. Reporting patient status to the RN and document pertinent observations in the medical record according to established procedures.
Set up patient care related equipment, such as hypothermia machines, oxygen delivery systems, portable and wall suction and assisting in monitoring proper functioning of equipment. Utilize glucose monitoring machines to collect glucose values on veterans when needed and refer deviations to appropriate nursing staff or other health care provider. Make observations of subtle changes in resident's condition, i.e. behavior shifts, mood, skin coloring, appetite, etc.
and takes action accordingly within the NA's assigned role. Documents care given, observations and responses on appropriate tools or forms. Respond appropriately to subtle patient changes by contacting the appropriate nursing personal. Administer complete tracheostomy care, including suctioning. Monitor and document patient intake and output reports deviations timely and accurately.
Perform nursing procedures within scope of practice and according to policies and standard procedures to care for the veteran for such procedures but not limited to enemas, catheterization, colostomy care, skin care simple dressing, application of braces and prosthetic devices. Work on small group activities with patients/residents according to the patients/resident treatment plan.
Recognize the residents' autonomy and promotes independence by means of respect, limit setting and individualized treatment. Actively participate in Treatment Planning Meetings. Utilize equipment to promote self-care. Verbally provide patient information to the treatment team for inclusion in the veteran/residents' plan of care, participates in treatment team meetings for resident care planning.
Enters information about the residents' condition in the chart, utilizing appropriate formats.
