Job Summary

Responsible for all elements of the nursing process when providing and/or supervising direct patient care. Responsible for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care by collaborating with members of the interdisciplinary team.

Major Duties

Provides competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Performs triage and assessment of patients on the team panel or unit making sure to provide prompt and appropriate treatment for Veterans health concerns to ensure care is timely. Provides care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Collaborates and coordinates care with interdisciplinary team members from other services within the department as well as services in the community. Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Participates in activities and strategies to sustain an evidence-based-practice culture. Provides peers with informal constructive feedback for improvement.
