Job Summary

The Registered Nurse Care Manager works at the Ashtabula Community Based Outpatient Clinic (CBOC). Incumbent will be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Incumbent will assess, plan, implement and evaluate care based on age specific components and adhere to established policies, standards of care and standards of practice.

Key Responsibilities

Major Duties Include but not limited to: Responsible for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, prevention and wellness based on the patient's goals, and supporting factors that impact customer satisfaction; Considers all characteristics of the individual, including age and life stages, state of health, race and culture, gender specific healthcare, values, and previous experiences; Administers medications and procedures per establishing policies and guidelines; Responsible for collaborating with services internal and external to the VA to facilitate care transition in order to manage and coordinate care to effectively meet the patients' needs; Provides consultation and education to nursing staff in areas such as new equipment, mandatory review, orientation and cross-training, and continuing education; Demonstrates clinical expertise and knowledge of health care programs to improve the quality of patient care and the use of resources to facilitate compliance with internal and external requirements and standards; Demonstrates leadership by serving as an advocate for patients.

Acquires and demonstrates strength in the areas of interpersonal relations, critical thinking, problem solving, and conflict resolution.

Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs; Leads initiatives within unit/department level shared governance; Supports and actively participates in initiatives such as High Reliability Organization (HRO), Patient Centered Care, Just Culture, and I CARE; Provide competent, evidence-based care to assigned patients and oversight of licensed vocational nurses/nursing assistants; Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.
