This position is located at the Fredericksburg Health Care Center (FxHCC) in Virginia. The Patient Aligned Care Team CPP functions at the highest level of clinical practice and works independently under their individual scope of practice to directly care for patients.
A CPP functions as a member of their assigned team(s) and serves as an advanced practice provider to initiate, modify, or discontinue medication therapy and as a consultant for medication therapy management services.
Major duties include, but not limited to: Clinical - Direct Patient Care Activities that require a scope of practice: 1. Core functions include face-to-face and virtual encounters with referred patients, for comprehensive medication management in practice areas identified in their individual scope of practice. 2.
Functions as an advanced practice provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. 3. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. 4. Follows-up with patients on lab or test results to discuss the plan of therapy, i.e.
changes in medication therapy, monitoring, and additional testing requirements. Clinical - Indirect Patient Care Activities: 1. Identify patients who may benefit from comprehensive medication management through population management reports during team huddles, referrals, and/or consults. 2. Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required.
Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. 3. Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. 4. Apply knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment recommendations based on objective laboratory findings.
Administrative Functions: 1. Participates in medication use evaluations activities, audits or quality management monitoring as designated by supervisor. 2. Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects. Assists in preparing for special accreditations and reviews (e.g. The Joint Commission, Inspector General, etc.,) and meets with reviewing/inspecting officials and pharmacy management.
3. Performs at least one hospital committee assignment per year, or attends professional meeting and presents a report to the facility's professional staff and documents committee contribution or impact of meeting. 4. Informs clinical coordinator of changes in nature of assigned duties which could affect clinical activities. Performs other duties as assigned.
