Job Summary

The VA Central California Health Care System (VACCHCS) is currently recruiting for a full time Pharmacy Technician with Pharmacy Service, located in Fresno, CA.

Key Responsibilities

The Clinical Pharmacy Technician shall provider and support the clinical operations of the pharmacy clinics such as pain, mental health, PACT, Pharmacoeconomics as well as support the outpatient pharmacy operations. All duties of the Clinical Pharmacy Technician will be performed under the direct supervision of a Clinical Pharmacist/Program Manager.

All assigned clinical activities and functions shall be completed in accordance with approved clinical pharmacy protocols or procedures under the clinical pharmacist supervision.

Assigned activities include: Technical duties, under the supervision of the clinical pharmacist and per clinic protocol, including completing electronic progress notes, scheduling pharmacy clinic appointments, initiating and conducting VA Video Connect (VVC) appointments with Veterans or representatives, and documenting data in VISTA/CPRS Clinical assessment a. Obtains and assesses information on compliance, labs, medication changes, side effects and general state of health.

b. For follow up patients, initiates electronic progress note in CPRS, including assessment for medication indication, duration, and side effects. c. Reviews medication changes (including any herbals, dietary supplements/OTC), adherence, and complications of therapy and reports findings to clinical pharmacist d. Routinely monitors laboratory data. Patients who have abnormal laboratory data shall be referred for evaluation according to clinic protocol. e.

Under the supervision of the clinical pharmacist perform initial medication reconciliation reviews f. Routinely performs patient medical chart, medication profile, and action profile reviews to identify poly-pharmacy situations. Reports identified patients to assigned clinical pharmacist for evaluation. Communicates with the clinical pharmacy specialist and with other staff (including providers and nursing personnel) any complications of medication therapy or other problems.

Assists with local and national Opioid Safety Initiative (OSI) monitoring. Reviews compliance documentation, last date of urine drug screen (UDS), last date state prescription drug monitoring (SPDMP). Results are consolidated and documented in CPRS for every opioid renewal request placed with the pharmacy call center.

If any above information is not present in the patient's chart or has expired, the Clinical Pharmacy Technician will place the appropriate alert to the clinical pharmacy specialist and or provider. The incumbent will also notify the patient of what actions need to be taken, how to complete them and if asked supply the supporting policies and reasoning behind the requirements. a.
