Job Summary

The VA Sierra Nevada Health Care System (VASNHCS) Physical Medicine & Rehabilitation (PM&R) Nurse Practitioner is a professional Registered Nurse (RN) and nationally certified Nurse Practitioner (NP), who uses the nursing process and evidence-based practice to work collaboratively with the team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient).

Key Responsibilities

Major Duties not limited to but may include: Applies the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to systems or processes at the unit level to provide patient-driven holistic care. Provide nursing and medical services to Veterans and their families/caregivers in both individual and group settings. Emphasize health promotion and disease prevention; diagnose and manage acute, chronic and episodic illnesses.

NP practice dimensions may include but are not limited to ordering, conducting, supervising and interpreting diagnostic and laboratory tests and prescription of pharmacologic agents and non-pharmacologic therapies. Teaches and counsels Veterans, families/caregivers and groups. Demonstrates leadership by involving the HBPC staff in providing customized, evidence-based patient-driven care.

Designs, implements and revises interdisciplinary patient-driven holistic care plan for life. Maintains privacy and confidentiality of all patient information, including electronic, print, and conversations. Provides patient-driven care in a safe, efficient and cost-effective manner, using evidence­ based principles. Serves as a professional leader in the health care delivery systems and embraces lifelong learning.

Requirements & Education

Note: Only education or degrees recognized by the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE) may be used to qualify for nursing positions.

The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration.
