Job Summary

The VA Northeast Ohio Healthcare System is recruiting for an Physical Therapist. The Physical Therapist will function within Home Based Primary Care (HBPC) Program provides comprehensive interdisciplinary care to veterans with complex medical and psychosocial conditions within the home care setting. HBPC therapist complete evaluations to determine the patient's function and safety in their home and community.

VA Careers - Physical Therapy: https://youtube.com/embed/Gi2hoFqIoqY The Physical Therapist duties include, but are not limited to: At the full performance level, Physical Therapists will function with limited supervision Utilizes current evidence based physical therapy practice to guide care. Modifies the treatment plan, therapeutic activities and interventions based on the changing needs, goals and performance of the patient. Modifies the treatment plan when either the established goals or timeframes for achievement of those goals has been attained. Provides clinical supervision to Physical Therapy Assistants (PTA's) and students as assigned. Consistently communicates and treats all internal and external customers (patients, family members, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Consults with patients and their families to educate them on their condition, instructs them in home exercise programs, and in the use of home medical equipment. Promotes effective and respectful communication with supervision and other disciplines. Work Schedule: Monday - Friday; 8am - 4:30pm Telework: Not Available Virtual: This is not a virtual position.
